Medical & Media Release
Medical Waiver/Release:
I, as a parent or legal guardian of the player, hereby give my permission for Women’s Apex Soccer Academy (W.A.S.A.) Staff to procure necessary medical treatment, until such time as I may be contacted, for my child/ward should they be injured or require medical attention while playing or practicing as members of Women’s Apex Soccer Academy. This permission shall be in effect for one year from date of signing. I, as a parent or legal guardian of the player understand that soccer is an active, physical sport, and that injuries can take place during play. I also do hereby assume responsibility for any accident or injury or death that may occur while my child/ward is participating in any event sanctioned by Women’s Apex Soccer Academy for ourselves, our heirs, executors and administrators, waive, release and forever discharge Women’s Apex Soccer Academy (W.A.S.A.) and its staff, officers, agents, employees, representatives, successors and assigns of and from all rights and claims for injury which may be sustained or occur during participation in soccer activities.
Photo and Media Release
AS A TERM AND CONDITION of registering my child as a participant in any one or more of the, soccer programs, including but not limited to leagues, camps, trainings, tournaments, events or cultural exchanges (hereinafter Women’s Apex Soccer Academy (W.A.S.A.)”), whenever and wherever occurring which are owned by, operated by, managed by or affiliated with Women’s Apex Soccer Academy (W.A.S.A.), IT IS EXPRESSLY UNDERSTOOD AND AGREED that I hereby (a) grant Women’s Apex Soccer Academy (W.A.S.A.) and their respective representatives, employees and vendors the right to take photographs of me or my child in connection with either one of our, or both of our, respective participation in Women’s Apex Soccer Academy (W.A.S.A.) soccer programming and (b) authorize the organization and their respective assigns and transferees to copyright, use and publish without any compensation whatsoever any such photographs in print and/or electronic formats with or without our respective names included therewith for any lawful purpose, including for example such purposes as publicity, illustration, advertising, promotion and Web and other Social Media content and usage.
